The Population Health Manager develops and maintains performance improvement initiatives and ongoing communication with physicians and administration regarding productivity, quality outcomes, patient satisfaction, appropriate utilization and all other population health initiatives. Directs the application of performance excellence and quality improvement principles and methods to improve organizational performance, including but not limited to: analysis of current performance to determine areas of strength and opportunities for improvement, and comparison with best practices and national standards.

Job Duties

  • Responsible for understanding the HLN suite of clinical programs, identifying practice opportunities and integrating HLN clinical programs, processes and resources into the practice workflows with the intent to optimize the patient experience, quality of care provided and positively impact resource utilization
  • Lead practice transformation including but not limited to practice observations, review of findings, best practice recommendations related to a practice’s readiness for value based care
  • Lead the physician practice HLN work by analyzing performance reports and data to inform decision-making, process, and program implementation
  • Monitor and implement educational and process interventions based on HLN, group, practice and individual provider level data, trends and identified opportunities
  • Testing and refining of workflows that support sustainable transformation and guide practices as they expand interventions to additional patient populations
  • Active participation in participating provider group quality meetings and performance improvement meetings with the intent to serve as the subject matter expert related to the value based contracts the respective groups are a part of at any given time
  • Provide for enhanced access to care and network integrity optimization
  • Demonstrates clear understanding and ownership of performance improvement impact on the overall health system goals, physician compensation, contract results, etc
  • Manages the review of data to identify performance improvement opportunities and make recommendations to HLN executives
  • Identifies opportunities to improve efficiencies for providers in the group

Minimum Requirements & Education

  • 5 years of professional-level experience in healthcare, OR a master’s degree
  • Bachelor's Degree
