Establishes implements and monitors a plan of care for the medically supervised patient in weight management program.  Works closely with the clinic staff, Clinic Coordinator, NP/PA and MD, to provide exceptional medically-related interventions, options and education for the individualized clinic patient.
Minimum Education MS Licensed Nurse Practitioner. Weight Program experience Preferred. 
Required Skills
Possess sound clinical, technical, critical thinking and teamwork skills required.
Must be skilled in physical and mental health assessment, diagnosis, treatment, pharmacology and advanced clinical nursing practice.
Possess the ability to work effectively with all members of a care team. Reports to Clinic Coordinator. 
Must demonstrate ability to recognize patients' individual needs based on medical conditions, limitations and planned procedures. 
Required Licenses 
Nurse Practitioners
Current Mississippi RN license required; certification in specialty area desirable.
Minimum Work Experience  At least 2 years’ experience in a clinical area and BLS or ACLS certification is required.  
Essential Functions
Works closely with the clinic staff, Clinic Coordinator, NP/PA and MD, to provide exceptional medically-related interventions, options and education for the patient.
Communicates with patient PCPs and other HCP regarding patient participation and progress in the program
Communicates with program staff as needed to provide a continuum of care for patients. 
Collaborates with program staff to facilitate the patient enrollment process by answering inquiries, orienting potential patients and scheduling induction appointments. 
Provides group health education as if relates to medical weight management or health and medical risk factors. 
Conducts periodic program evaluations, including the yearly Health Risk Appraisal project, and disseminate results. 
Identifies appropriate health risk appraisals and methodologies to incorporate into clinic services. 
Maintains knowledge of current trends in the field of obesity treatment. 
Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. 
Participates in regular department meetings and other related meetings as indicated.
Facilitates implementation of evidence-based standardized care.
Establishes a prioritization method for assessing and following all patients.
Identifies performance improvement areas and leads or co-leads improvement strategies to address and improve performance.
Functional Demands
Prolonged periods of standing, walking, or sitting while on duty.
Lifting, pushing, and pulling up to 50 pounds with or without assistance.
Transferring, repositioning patients.
Reaching, stooping, bending, kneeing, and crouching for patient care functions.
Visual and auditory acuity and manual dexterity essential to performing designated duties required.
Optimal auditory acuity required.
Manual dexterity involving the handling of equipment and instruments or needles is essential to performing assigned duties.
Ability to perform effectively in a stressful and fast-paced environment.
Ability to pass all required health and other screening tests.
Physical conditions are clean, neat and well lit. Maybe subjected to unpleasant sights and odors, stressful situations and hazardous or infectious agents where judgment as to precautions need to be taken is essential. Climate control and ambient temperature variances may be experienced. Exposure to limited amounts of hazardous chemicals or substances and infectious disease processes are a possibility.  Direct contact wi
th blood or other body fluids to which universal precautions apply. Personal protective equipment is made available and must be worn.
