Medication Reconciliation Nurse
Reconciles patients' medication regimens during the admission process. Medication reconciliation is defined as obtaining a complete and accurate medication history with the involvement of the patient and/or family; comparison of a patient's home medications to the medications ordered on admission and identification of discrepancies, followed by a clarification of any noted discrepancies.
Degree from an accredited school of nursing.
Minimum Work Experience
Requires relevant clinical experience, requires knowledge of hospital computer systems and computer-assisted devices. Requires problem-solving skills and ability to make independent judgments. Must have good verbal and written communication skills. Must have ability to prioritize and ability to be involved in multiple tasks and projects. Requires ability to work with others and requires close attention to detail.
