Medication Reconciliation Nurse
|Reconciles patients' medication regimens during the admission process. Medication reconciliation is defined as obtaining a complete and accurate medication history with the involvement of the patient and/or family; comparison of a patient's home medications to the medications ordered on admission and identification of discrepancies, followed by a clarification of any noted discrepancies.|
Requires current state of Mississippi license as a Registered Nurse. Requires relevant clinical experience, requires knowledge of hospital computer systems and computer-assisted devices. Requires problem-solving skills and ability to make independent judgments. Must have good verbal and written communication skills.
