Ensures safe and effective transition of patients across the care continuum while promoting high quality outcomes. The Transitional Care Manager provides leadership and oversight to the transition of care process as patients transfer from hospital to post-acute care in the community. Reports to the Director of Care Management.

Minimum Education: Graduate of an accredited school of nursing or social work; BSN or MSW required.

Minimum Work Experience:  Must have 3 years of clinical experience in case management or social work required, preferably hospital experience with discharge planning.  Current licensure as a Master Social Worker or as a RN in MS is required.

Required Skills: Must be able to demonstrate the ability to analyze data, identify significant problems, and recommend appropriate avenues for corrective action or follow-up. Employee must be able to demonstrate the ability to recognize patients’ individual needs based on medical condition, age, limitations and planned medical care. Requires the ability to maintain records and to use independent judgement along with knowledge of community resources and customer relations skills. Employee should be knowledgeable on community resources and demonstrate ability in building strong relationships with community partners.
